At Oak View, we believe in a patient-centered approach to care. The preservation of dignity, self-respect, patient’s rights and privacy should be held in the highest regard within a caring environment. Choosing a nursing center for yourself or a loved one is a very important decision. It is important to discuss options thoroughly with the individuals involved, family members and personal physicians. Ultimately, your selection of a long term care provider should lead you and your family to a personal relationship with the individual center that encourages your participation, discusses mutual expectations and welcomes your opinion at all times. We would like to share with you some information about our center and the services that we offer.

  • Staffing Our center believes that our associates are the heart of what we do each and every day.

  • Behavioral Health Our center offers a behavioral health program that allows patients to maximize and enhance overall wellness in a professional and caring environment. An individualized program is developed to meet the specific needs of the patient’s diagnoses and clinical manifestations.

  • Assessment & Care Plan Continuum Each quarter, a patient and loved one will be asked to participate in a care plan conference. We recognize that the patient and the family are the best resources for helping us to identify patient needs. We will always invite your input and welcome recommendations.

  • Family Council Families of our center are encouraged to become involved in Family Council. The goal is that the Family Council and the Center will work together as a team to provide the best care, services and living environment possible.

  • Rehabilitation Services Our center is committed to offering quality physical, occupational and speech-language pathology services. We are pleased to offer therapy services provided by Integra Rehabilitation. We work with Integra Rehabilitation to create an interdisciplinary team approach that involves each therapist in the care process from beginning to end.
  • Physician Services Each patient’s care and treatment will be overseen by a physician of his or her choosing, as well as consultant physicians as needed.

  • Restorative Program Our center offers a restorative program intended to improve both the physical ability (such as range of motion for joints and general mobility) and mental function (such as memory improvement, comprehension, and problem-solving skills) Whether in a group session or individually, our patients are encouraged to achieve and maintain their highest functioning level and enjoy doing it!

  • Resident Council Patients are encouraged to become involved in Resident Council. These meetings provide a forum for patients to share ideas, voice concerns, or make any suggestions that they feel would enhance the services we provide.

  • Activities Program The Activities Program at our center offers a wide variety of events to stimulate the interest and creativity of our patients. From exercise groups to reminiscing with fellow patients in a memory improvement exercise, our associates are dedicated to providing meaningful and purposeful activities to keep the patients engaged and active.

  • Salon Services Salon services such as hair and nail care are offered just steps from our patient’s living areas.
