No. Your agency will be included in our national database and shown on our site at NO COST. There are additional ways for you to enhance your business, should you choose to. We offer providers additional exposure through mini website hosting and advertising options at incredibly low prices. Who should complete the survey?
We request the Primary Contact individual from all providers. Please enter one survey for one business location. If you have multiple branches/offices, our survey tool will assist you in uploading an Excel spreadsheet of your providers.
How long will it take to complete the survey?
The survey should take about 10 minutes to complete. You can also come back later and update your listing on our site without starting a new survey.
Can I receive a printed survey?
You can send us an email requesting a printed survey. From there you can email, fax, or mail it back to us.
If I cannot complete the survey in one setting, can I come back and complete it later?
Yes. Our step by step process allows you to save your responses on each page. Using your username and password, you can come back later and pick up where you left off.
How can I change my provider information on the survey and listed on your site?
By using your username and password, you can update your information anytime from http://www.tlchoices.com/hhsurvey.
What do you do with the survey information?
The survey will go through a quick review before being imported into our database. Once approved, the results will be published publicly on your provider profile page, accessible through the TLChoices website. Your business will also appear in searches performed by hospital discharge planners using our Care Finder-Pro tool. You may receive free qualified referrals from both these sources.
We will notify you by e-mail with an attached URL to your profile when it has been approved, posted and live on our website.
What will the consumers do with the results?
Discharge planners, individuals and their families will use our tools to search and select from a thorough list of medical and personal care needs. From here they will then contact providers matching their needs by phone, fax, or email.
